Social determinants of health and the $1.7 trillion opportunity to slash spending

Cliff Frank, Interim Executive Director of Shore Quality Partners, was a featured speaker at the HIMSS Pop Health Forum in Chicago, IL, on October 2, 2017. As one example of how Shore is using HealthEC’s population health management solution to reduce costs, investments in non-billable services to address social determinants of health has reduced the admission rate among diabetics from 68 per thousand to 42 per thousand. Spending has also decreased from $2,800 to $1,300 per member per month, leading to shared savings for the organization.
